The first tab of the case information screen. Required fields are indicated with an asterisk (*).
When data is changed on the case screen, the Save button will turn red to indicate that changes have not yet been saved. Once the record is saved either by clicking the save button or changing tabs in some instances, the save button will revert back to white.
This screen contains many drop-down lists that are common to the system. To learn how these operate, see Drop Down Lists under Common Program Features.
| Case / Acct Number | Each case is assigned a unique code or account number. This field can be left blank and the system will generate a code automatically. You may want to use it to reference an account number in another billing system. |
| Description | Brief description for the case. This is useful on screens that list patient cases to help ensure that the proper case is selected for scheduling or EMR note entry. |
| Inactive / Discharged | Indicates the case is no longer active. If using the EMR system, this box will get checked automatically when a provider creates a Discharge Summary. |
| Referral Date | Date of the case referral |
| Referral Source | Categorical source of the referral. Not to be confused with referring physician. Categories may be sources such as MD, Friend, Newspaper Ad, Current Patient, or any other source you decide to add to the list. Referral sources are found under the Address List and are assigned a category of "Referral Source". |
| Referring Physician | Physician who referred the case. Since this is a required field, a physician named "None" can be added to the system to indicate there was no referring physician. |
| Provider | Provider / therapist assigned to the case. If entered, there is an option to verify that this case is only scheduled with the provider assigned. See Scheduling Options for more information. |
| Facility | Facility or office where the patient will be seen. |
| Patient Type | Type of patient for this case. Examples include PT, OT, Speech, etc. The clinical statistics of patients seen, cancellations, etc. will be broken out according to each patient type. Additionally, each patient type added to the system can be customized to create different documentation layouts. |
| Diagnosis | Patient primary diagnosis, if known. This diagnosis gets carried over to the patient Evaluation. If it is left blank, it will get populated when a provider / therapist enters a diagnosis on the patient Evaluation. The evaluation screen allows up to 4 diagnosis entries. |
| Date of Onset | Date of injury or onset This also gets carried over to the patient Evaluation. If it is left blank, it will get populated when a provider / therapist enters a Date of Onset on the patient Evaluation. |
| Co-Pay | Amount of patient's co-pay. If entered, the scheduler will alert users about co-pays due. |
| First Visit | Checked automatically when adding a new case. Lets the scheduler know that an appointment being scheduled will be the first one for this case. This allows the person doing scheduling to allow more time for an evaluation. Once the patient has been scheduled, this gets unchecked automatically. Through normal use of the system, this box should not have to be checked. |
| No Charge | Indicates a patient that should not be counted in clinical statistics such as Daily Numbers. No Charge patients will appear in Italics on the Appointment Book. |
| Do Not Double Book | Prevents users from scheduling other patients in a same time slot that this patient occupies or scheduling this patient in a time slot that is already occupied. |
| Medicare | Indicates a Medicare patient. If selected, the "Units to Bill" calculation will follow the Medicare 8-minute rule instead of the rules established on the Procedure Codes screen. |
| Auto | Indicates auto type case. Used mostly for the Altapoint interface. |
| Worker's Comp | Indicates worker's comp type case. Used mostly for the Altapoint interface. |
| Blue Cross / Blue Shield | Indicates Blue Cross / Blue Shield type case. Used mostly for the Altapoint interface. |
| Color Code | Used to change the text color of the case on the Appointment Book. Clicking the palette will access the color coding management screen. Click here for more information on color coding. |
| Primary Physician | Patient's primary physician if different than the referring physician. |
| Other Physician 1, 2 | Additional fields for specialty or other physicians |
| Last MD Date | Last time the patient was seen by their physician. |
