Case - Insurance Information

There are 3 different tabs for insurance information on the case screen. Insurance Policy 1 for the primary insurance, Insurance Policy 2 for secondary, and Insurance Policy 3 for tertiary. All of these tabs function the same way.

Insurance Company Name of the insurance company.
Insurance Type Type of insurance.
Policy Holder Name of insurance policy holder. More details below.
Relationship Relationship of the policy holder to the patient (self, spouse, child, other)
Policy Number Insurance policy number
Group Number Insurance group number
Effective Date Effective date of the insurance policy
Co-insurance co-insurance due
Deductible Insurance deductible
Restrictions Any insurance restrictions. If anything is entered into this field for the primary insurance, it will also appear at the top of a Daily Visit Note screen when a therapist enters charges for reference purposes.

By default, the patient is the policy holder and the relationship is "Self". To change the policy holder, click the button beside the policy holder drop down box and the screen below will appear. Select "Self", "Patient", or "Guarantor".
